If you're planning to DIY your own face mask, you've probably asked yourself these questions:

I want to answer these questions using information directly from hospitals and the Centers for Disease Control and Prevention (CDC). Find out how medical professionals think you should make your mask.

Types of face mask designs:
The 2 most popular designs are pleated surgical-style masks and olson masks.
Olson masks are a more fitted style. They can fit over N-95 masks according to Cameron Memorial Community Hospital.

Here's a comparison table to help you choose quicker.
Not all the masks are in this table, however. There are 28 other designs that are not included because they're very similar. I have a brief summary about them instead.

This mask was "developed by clinicians from UnityPoint Health" (released 23 March 2020). They are a nonprofit health system in America.
The design includes:
- Cotton (fabric.com)
- Elastic ear hooks using hair ties (Amazon).
- A filter pocket.
- Double-sided skin adhesive (Amazon) to create a seal around the nose and mouth.

The Craft Passion blog initially made this face mask tutorial in 2013.
This design includes:
- Cotton (fabric.com)
- Elastic around the head or elastic ear hooks (Amazon link).
- An option to include a filter pocket.
- An option to include a nose wire. Use 16 gauge craft wire (Amazon). You'll want to cut and bend the ends with jewelry pliers (round nose) and cutters (Amazon).
- 4 sizes: men, women/teens, and 2 patterns for children (3-6 yrs, and 7-12 years).

"Bronson's Patient & Family Advisory Counsel along with clinical staff members have collaborated on a mask template to best fit current needs." It comes in 2 sizes: standard and large. A PDF pattern and instructions are available.
"Emergency room physician Dr. Andrew Pizza came up with the idea in a staff meeting when talk turned to the difficulty of finding masks for patients being discharged.
Staffers found a pattern for making them, available online [by Craft Passion], and then got a second opinion.
"We got it OK'd by infectious disease specialist Dr. Faith Joubert," said Jennifer Berger, the hospital's manager for marketing and communications."

- UH Hospitals who have 5 medical centers. They're trying to create 100,000 masks in 2 weeks.
- Gritman Medical Center. They have adapted the face mask pattern and made it bigger. They said "the very high heat wash that Gritman is putting these finished masks [through] is shrinking even pre-shrunk fabrics. This is the updated, slightly larger version." They have also included a sewing pattern for a filter.

7. Pleated surgical-style mask with ties – by Kaiser Permanente

This mask was designed by Kaiser Permanente, a healthcare provider in 8 states: "Kaiser Permanente has developed step-by-step instructions for making non-clinical cloth masks at home." It was released 27 March 2020.
The design includes:
- Cotton or cotton-blend fabric. It can be a woven or knit. Fabric.com has a big range to choose from.
- Non-stretch ties around the head. You can make these yourself or buy pre-made bias binding (Amazon).
- 2 layers of fabric.

- Nose wire. Use 16 gauge craft wire (Amazon). You'll want to cut and bend the ends with jewelry pliers (round nose) and cutters (Amazon).
- Filter pocket.
"The project has been designed by our staff here at the practice, notably Jen Hymas and Dr Parrish.
They have based the design on the shape of the paper masks that were issued to us by NHS England. They have made a few prototypes which are quite comfortable to wear."

13. Both face mask styles & fit tested – University of Florida Health

About this mask:
The University of Florida Health (UF) designed 2 mask styles: a pleated style and rounded style.
What makes these masks stand out is that they have passed fit testing and use an interesting material.
The designs include:
- Medical grade fabric: Halyard H600. (Note: there's some debate about this material.)
- Ties. You can make these yourself or buy premade bias binding (Amazon).
- Nose wire using 16-gauge craft wire (Amazon). If unavailable, use 20-gauge (Amazon). You'll want to cut and bend the ends of the wire with jewelry pliers (that have tapered round ends) and a cutter (Amazon).

This Youtuber and medical worker does her own fit test of the mask and then starts sewing it at 17:15.
Fit tested
Unlike many masks, these have been fit tested.
Fit testing checks whether a mask fits without gaps and creates a seal. 3M says "if the seal is inadequate, contaminated air will take the path of least resistance and will travel through leaks in the face seal".

"The masks have been personally N95 fit tested by more than 20 volunteers. In the people who tested it, the mask passed the sweet or bitter taste test akin to what is used for N95 respirators. However, that does not certify it as an N95 respirator.
If you construct these masks or receive them according to our design, they should be fit tested to your face. If it passes fit testing, please use it but do so with the caveat that these are not commercially produced, third party-certified respirators.
These are community-produced filtration masks created to be extenders of our supply with some increased protection over a standard surgical mask. They cannot be regarded as frontline primary COVID… protection."

Halyard H600 fabric
These masks are made using Halyard H600 fabric. There are claims that it has filtering properties, although there's some debate about this.
UF said it's "typically used to wrap surgical instrument trays in medical settings. The Halyard H600 is the most protective grade of the material."
It doesn't look like you can wash it. UF are cleaning them with specialist equipment: an autoclave and UV light sterilization.
They said "if you are making a mask at home for home use, you can sterilize it by placing it in an oven at 165°F for 30 minutes. Other options may be available."
"The material is thought to be superior to the common surgical mask in its ability to block aerosols and droplets, including water, bacteria and other particles.
Although the Halyard H600 material has undergone rigorous testing and has been assigned a filtering (N) rating by the manufacturer, it hasnot, to our knowledge, been tested for use as a breathed air filter by any regulatory body.
Although it isnot rated as an N95 for filtering breathed air, we do subject it to the same particle filtering test we use for commercial face masks and find that with a properly fitted, snug mask, it appears to provide similar particle filtering properties as commercial masks.

Olson style masks
- This olson mask was designed by Monadnock Community Hospital, NH (released 23 March 2020). The printable sewing pattern is included. Each step in their instructions is explained well with pictures.
- Allina Health designed an advanced olson mask sewing pattern. It includes an adjustable nose, pleats, and ties under the chin.
- John Muir Medical Center have made an olson face mask. They have included printable sewing patterns.
- University of Utah Health designed an olson mask. They included sewing patterns in 3 sizes (small, medium and large) and illustrated instructions.
- Promedica has instructions for an olson mask with elastic ear hooks.
Pleated surgical-style masks
- Childrens Wisconsin created a surgical-style mask. They have printable instructions with pictures, a pdf sewing pattern, and a staff tutorial video.
- Valley Health created a surgical-style mask with ties. PDF instructions and videos are available.
- Cayuga Health System has instructions for a surgical-style mask. It's not very visual though.
- Vanderbilt University Medical Center has instructions for a surgical-style mask. There's no pictures or videos (that I've found), so it's not the easiest tutorial to follow.
- Michael Garron Hospital created a surgical-style mask with elastic ear hooks. They are trying to get 1000 sewn mask donations a week.
- Brookings Hospital, SD asked donors to make Sweet Red Poppy's surgical-style mask with ties. Printable instructions are here.
- RWJ Barnabas Health created a surgical-style mask. Their instructions have illustrations of each step.
- Saint Lukes created a surgical-style mask. The instructions have helpful pictures.
- OSF Healthcare made a surgical-style mask.
- Northside Gwinnett Hospital, GA designed a surgical-style face mask with ties. PDF instructions and a sewing pattern are included. PS. I don't think they're accepting sewn mask donations anymore.
- St. Francis Medical Center has instructions for a surgical-style mask.
- Mclaren made instructions for a surgical-style mask. They have instructions with pictures and a printable sewing pattern. This was the pattern they requested in a mask request database, however, their website asks donors to use the mask pattern by Make Victory Masks. The site may be more up to date.
- Mercy One has a really helpful video tutorial on their surgical-style mask design. Note: they're not taking any more sewn mask donations. They have received enough. They have taken down their donation page but the video is still live.
- Community Health Network approved 2 surgical-style face mask tutorials from Youtube (by Homemade on our homestead and Erica Arndt).
- Great Falls Clinic approved this surgical-style mask by Youtuber EasyToSew. It has elastic ear hooks, a filter pocket and adjustable nose wire. They also linked to this simpler surgical-style mask with ties by Youtuber Benjamin.
- Providence St. Joseph Health created a surgical-style mask design with ties for their 1 million masks challenge. They're no longer accepting sewn mask donations, but their video tutorial is still live. It's great for beginners; each step is explained really well.
- Day Kimball Hospital, CT made instructions for a surgical-style mask. There are no visuals though (as far as I know).
- Owensboro Health Regional Hospital, KY approved this surgical-style mask by the Stitching Scientist blog.
- Emory Healthcare approved a surgical-style mask with elastic ear hooks designed by Button Counter. There's also a printable pattern.
- Melrose Wakefield Hospital, MA approved this surgical-style design by Youtuber Backpack Tutorial.
- Kettering Health, OH operates 9 hospitals. They asked donors to follow this surgical-style design by Made Everyday. (Note: their request for homemade masks is not currently on their website, but can be found in this request database).
- Joann's surgical-style mask with elastic ear hooks has been approved by: Foothill Presbyterian Hospital, Inter-Community Hospital, St. Vincent's Medical Center, Trinity health of new england, Eskenazi Health, and Virginia Cancer Specialists.

Most of these masks require:
- Cotton fabric (fabric.com). Use a tightly woven fabric with a high thread count, if possible.
- Ties. You can make these yourself or buy pre-made bias binding strips (Amazon).
- Elastic. Some hospitals recommend flat elastic, normally 1/8″ to 3/8″ in size. An alternative is ponytail holders with no metal (Amazon links). Wide elastic and rounded elastic might become uncomfortable around the ears.
- Nose wire. The University of Florida Health recommends 16-gauge craft wire (Amazon) to improve the fit of your mask. If that's not available, use 20-gauge (Amazon).
- You'll want to cut and bend the ends of the wire with jewelry pliers (that have tapered round ends) and a cutter (Amazon).
- A rotary cutter, long ruler, and cutting mat (Amazon) will make cutting out masks much faster and more accurate. I would only bother getting these if I was making a lot of masks, however.

Are these face mask designs CDC compliant?
CDC stands for the Centers for Disease Control and Prevention. It's a US organisation that sets guidelines for the medical community to follow.
In their "Strategies for Optimizing the Supply of Facemasks" document, they say that healthcare personnel (HCP) could consider using "homemade masks (e.g., bandana, scarf)…as a last resort".
From this my understanding is that any face covering made from cloth is fine.
The CDC released more guidelines on DIY face coverings on 4 April 2020.
"Initially, the CDC did not advise people to wear face masks in public. One reason was to protect the supply for use by health care professionals and first responders. That remains a top priority.
Another reason was because face masks like the ones discussed here don't offer significant protection for the person wearing the mask.
Recently, however, medical professionals have determined that COVID… can be transmitted by a person without symptoms. That could be a person who is sick but showing no symptoms, or even a person who has been infected but not yet fallen ill."

UPDATE: This page on the CDC website was last reviewed on 13 April 2020. The bandana tutorial has been changed to include NO COFFEE FILTER. As far as I can see, they haven't explained why the filter was removed.

Elastic advice
Many hospitals have asked sewists to use flat elastic. Normally a 1/8″ to 3/8″ size.
If you don't have this, you could cut braided elastic to the correct width.
Using wider elastic than this could be uncomfortable to the wearer. Rounded elastic might also be uncomfortable around ears.
Another backup option is using ponytail holders, but not ones with metal.
Elastic advice (evidence):
"Flat elastic in 1/8", 1/4", or 3/8" widths will work with the mask pattern shown in the video link [by Deaconess Health].
White is preferred, but other colors are acceptable.
Rounded elastic is harder to sew securely in place, as well as less comfortable to wear around the ears.
What length elastic should be used?
6.5" for the Small, 7" for the Medium, and 7.5" for the Large works well for most people.
I can't find the appropriate size elastic. Can I use larger elastic?
No, larger elastic would be very uncomfortable behind the ears.
The good news is that wider braided elastic can be cut to the appropriate width using scissors or a rotary cutter. This only works with braided elastic, NOT knit elastic."

I cannot find elastic anywhere; what can I do?
Locally stores have depleted their stock. Many sewing groups are sharing elastic but also using pony tail holders and scrunchie pony tail holders instead.

"Not all facemasks can be re-used.
– Facemasks that fasten to the provider via ties may not be able to be undone without tearing and should be considered only for extended use, rather than re-use.
– Facemasks with elastic ear hooks may be more suitable for re-use."

Ties advice
Fabric ties are a recommended alternative to elastic. They also provide a more universal fit. A downside is that they could get tangled and knotted when laundered.
Advice about ties (evidence):
"If flat elastic is not available, fabric ties can be sewn on the mask, one tie on each of the four mask corners.
Ties should be approximately 1/4" width, sewn so there are no frayed edges, and approximately 15" for each corner."

Ties should be approx. ¼ inch in width, sewn in such a way that there are no frayed edges, and long enough to be tied into a bow on the top of the head (for the top corner and behind the head for the bottom corner).

How to clean & disinfect your DIY mask:

"Recommended cleaning steps
STEP 1: Remove and discard the filter. Spray mask front and back with hydrogen peroxide or equivalent quality solution.
STEP 2: Gently kneed with warm, soapy water. Do not soak. If placed in washing machine, do not place with any other items.
STEP 3: Allow to dry on clean, sanitized surface that is free from high-touch areas."

How do I clean a homemade cloth mask?
Homemade cloth masks should be properly cleaned before they are reused.To wash, launder masks in hot water (160°F) using soap or
detergent that leaves no residue.Rinse well with fresh water and hang to dry.
You may also disinfect masks by soaking them for 5 minutes in a solution of 2 tablespoons of bleach per quart of water or 1/3 cup of bleach per gallon of water.

Sam Caruso Jr., hospital director of business development, wants to be doubly clear that the masks are not for hospital staff.
"The purpose of giving these to people to take home is to use to avoid the spread of the disease," which can be passed on by heavy droplets, such as from coughing or sneezing, he said.

How is Owensboro Health using the masks?
We will be using the cloth masks for situations where a surgical or N95 mask would not be required. This extends the supply of our disposable surgical masks for those times when procedural care is required.
While we are not out of our current supply of surgical masks we know getting new additional masks will be difficult because the national supply is running out.
The fabric masked could be used over the surgical masks to keep them from getting soiled and extend their use.

Do cloth face masks really work?
Note: these research conclusions are NOT about Covid…
"Washable, woven cloth masks have been used in hospitals in some countries. Also, in the absence of any alternative, some members of the public may improvise respiratory protection out of t-shirts, scarves, or other cloth.
Given the lack of sufficient data either supporting or refuting the effectiveness of woven cloth masks and improvised coverings in blocking influenza transmission, the committee hesitates to discourage their use, but cautions that they are not likely to be as protective as medical masks or respirators.
We are concerned that their use may give users a false sense of protection that could encourage risk-taking."
